Charting the LifeCourse is a collection of planning materials to help people with disabilities and their families think about what they need to know at any age or stage of life, including how to find or develop supports and discover what it takes to live the lives they choose.

Use the profile page to share information about the people you support with school staff, medical staff, child care providers and others. 

Use the life trajectory worksheet for planning and goal setting, such as IEPs at school or waiver service planning.
