WHAT'S CHANGING?
On April 1, 2025, HealthPartners will no longer offer their Special Needs BasicCare (SNBC) non-integrated program called "Inspire".
If you already submitted a SNBC Choice Form to enroll into a different plan, you may receive a new notice by mail with changes to your enrollment.
WHO WILL THIS CHANGE AFFECT?
If you are currently enrolled in the HealthPartners Inspire Special Needs BasicCare program, your options will change.
- If you live in Hennepin County, you will automatically be enrolled to Hennepin Health coverage on April 1, 2025.
- If you live in Anoka, Benton, Carver, Chisago, Dakota, Ramsey, Scott, Sherburne, Stearns, Washington or Wright county, you will automatically be enrolled to Medical Assistance Fee-for-Service (FFS) coverage on April 1, 2025.
This health plan change does not affect your eligibility for Medical Assistance health care coverage.
If you are eligible for Medicare, the change to your coverage on April 1, 2025 will not affect your Medicare Part A, B or D coverage.
If you are currently receiving disability waiver services including Brain Injury (BI), Community Alternative Care (CAC), Community Access for Disability Inclusion (CADI) or Developmental Disability (DD) waivers, personal care assistant (PCA)/Community First Services and Supports (CFSS) or home care nursing (HCN) services, you will continue to receive those services without interruption. These services are already paid through MA Fee-for-Service. You can contact your waiver case manager with any questions.
WHAT DO YOU NEED TO DO?
If you are satisfied with the changes, you do not need to do anything.
It is recommended that you check with your providers to make sure they will accept your new coverage starting April 1, 2025. Also, review your medications to confirm they will still be covered.
You can do this by:
- Calling your providers directly to ask about your coverage
- Using the online Minnesota Health Care Program (MHCP) Provider Directory list to verify in-network providers
- For Hennepin County residents, contact Hennepin Health Member Services at 612-596-1036 (800-647-0550), TTY 711 (800-627-3529) for assistance
- For all other counties affected, contact Health Care Consumer Support (HCCS) at 651-297-3862 for assistance
- For questions about Fee-for-Service pharmacy covered services, contact Prime Therapeutics at 844-575-7887
