Does the youth know how to access and manage health care benefits?

Ensure that each youth:

  • Has sufficient medical benefits to cover medical costs
  • Has access to their medical benefit cards
  • Understands how to contact their benefits provider with questions about coverage
  • Has a primary medical and dental provider
  • Knows their providers and how to schedule appointments as needed
  • Awareness: Understand current health care needs and benefits.
  • Exploration: Identify personal strengths, preferences, interests and needs related to health care and benefits. Explore how current health care and benefits could change after high school.
  • Preparation: Plan for health care and benefits after high school. Practice skills related to managing health care and benefits.
  • Implementation: Mange health care and benefits as independently as possible.

Visit Got Transition for support moving youth and young adults from pediatric to adult health care. Got Transition is a national resource center that aims to improve transition from pediatric to adult health care through evidence-based strategies for health care providers, youth, young adults and their families.

The health care transition readiness assessment is a questionnaire that helps youth identify areas in personal care and health care to learn more about. 

My health care team and support needs is a Charting the LifeCourse tool youth can use to log and easily share their health care team and needs. 

Today's health care visit from Charting the LifeCourse teaches youth how to manage their own health care visits.
