All transition professionals are asked to align their work to Minnesota’s Youth in Transition Framework and strengthen their partnerships and delivery of high-quality transition programs and services.

The resources below are meant to support your ongoing work to align and reach improved youth outcomes. Use and adapt them in ways that makes sense for you and your organization/team. The results and activities are yours for your use — there are no right or wrong answers.

Tools to support your work

Activities to help you align your work to specific aspects of the Framework

This activity assists in ensuring that all the needed people are participating in a regional/local transition planning team and that all team members understand the roles and services for school district/charter schools, Vocational Rehabilitation Services (VRS), State Services for the Blind (SSB), and waiver programs.

This activity assists you and your organization in reviewing youth employment and postsecondary education outcomes at local, regional and state levels.

The Framework's third shared practice is the youth planning process. A key tool to use during this process is the Transition/Pre-ETS Inventory, which can be completed in a meeting with the youth, family, and other support team members. It allows for the identification and prioritization of the youth’s strengths and needs and which learning stage or stages they are at within each of the learning expectation/Pre-ETS topics.

The youth can be supported in uploading the completed inventory into their My Vault account and then sharing with their support team.
